Prevea Health is an integrated multi-specialty group. With primary care providers and exceptional specialists, we offer services that can't be found anywhere else in Northeast Wisconsin. We also provide our high-quality health care services in Western Wisconsin's Chippewa Valley region. We partner with six hospitals: HSHS St. Vincent Hospital and HSHS St. Mary's Hospital Medical Center in Green Bay; HSHS St. Nicholas Hospital in Sheboygan; HSHS St. Clare Memorial Hospital in Oconto Falls; HSHS Sacred Heart Hospital in Eau Claire; and, HSHS St. Joseph's Hospital in Chippewa Falls. We share one combined electronic medical record with our hospital partners, so wherever a patient is seen--in one of our health centers or hospitals--his or her com...plete medical record will be readily available to all treating providers. This ensures the highest continuity of integrated care.
