Description
Under the direction of the Medical Director/Supervising Physician, the Palliative Care Nurse Practitioner serves on an interdisciplinary consulting team consisting of physicians, nurse practitioners, social workers, volunteers, etc. In collaboration with the members of the interdisciplinary team, the Nurse Practitioner aids in the development of care plans for patients and families facing serious illness and end-of-life decisions and manages associated complex symptoms. The Nurse Practitioner is expected to place emphasis on educating patients, providers and trainees to support the growth and understanding of palliative care services. The NP will be expected to work towards Advance Certification in Hospice and Palliative Care after starting in the program.

The Palliative Medicine Program at Stony Brook University Hospital resides in the Department Medicine, Division of Hematology and Oncology. This is an exciting time to join our growing and very dynamic interdisciplinary team!

Duties:

  • Conduct comprehensive or episodic health assessment of acute medical patient/family's with real or potential acute and/or health problems which may include the psychosocial, cognitive and personal assets and limitations of the patient/family, with an emphasis in palliative care and end-of-life care.
  • Establish medical diagnoses; evaluate diagnostic studies; prescribe and regulate medications; monitor therapeutic procedures; assess and/or manage follow-up plans within the protocols and practice privileges approved by supervising physician and consent with established policies and procedures.
  • Record and document health appraisal data necessary to maintain, coordinate and/or expedite required services.
  • Maintain ongoing communication with physician collaborator and associate director of nursing relative to select aspects of patient care or administrative issues.
  • Act as resource and consultant to health care staff.
  • Develop, conduct and evaluate formal and/or informal continuing education programs in the area(s) of expertise at the University Hospital and its community.
  • Participate in the development and revision of nursing standards of care.
  • Participate in nursing, medical and administrative committees.
  • Maintain and enhance a current level of knowledge relative to the professional practice, as well as, continuing education requirements necessary for licensure and certification. Attend educational conferences.
  • Participate in the research process by contributing ideas, collecting data, sharing knowledge and applying new findings to practice.
  • Participate in the development and review outcome data to measure the quality of patient care.
  • Other duties as assigned by palliative care physician and associate director of nursing, medicine/surgical/oncology services.



Qualifications
Required Qualifications:

Current New York State RN license. Master’s degree in nursing. New York State license and certification as a Nurse Practitioner in Acute Care, Palliative Care, or Adult Health. Minimum of 3 years direct patient experience to include one year of experience as a Nurse Practitioner in Acute Care, Oncology or Palliative Care setting.

Preferred Qualifications:

Certification in Advance Practice/Hospice and Palliative Care. Inpatient Palliative Care experience.
