The Adolescent and Young Adult (AYA) program is focused on improving the quality of life and outcomes for patients with cancer, specifically adolescents and young adults ages 15 – 39 years. The AYA Patient Navigator is a central member and change agent of the AYA Program team who follow patients through the continuum of care from diagnosis, through treatment, and into survivorship; schedules visits and coordinates care with subspecialties; informs AYA of what to expect during treatment; screens for barriers to care and treatment compliance; provides patient education on disease and treatment processes; initiates screening for clinical trials; and evaluates fertility preservation requirements.
Registered Nurses with current RN licensure in the State of Texas, BSN required/MSN or a Master in Social Work.
Oncology experienced required
Post Master's Degree experience in a health care setting in oncology is preferred.
LICENSURE/CERTIFICATION Registered Nurse - Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required. National certification (e.g. CCRN, RNC, CEN, CNOR, OCN, ANCC, CAN, CPAN, CFRN, etc.) in related field is preferred. Must have a current AHA BLS Healthcare Provider card. OR Social Worker - Current state licensure from the Texas Department of Human Services is required.
